Does it work on scanned PDFs?

Only if the PDF already has a text layer (i.e. it was OCR'd before). For raw scans without a text layer, you'll get an empty result. OCR is on the roadmap.

Does it preserve layout?

Partially — text from the same line stays on one line, but multi-column layouts may interleave. The goal is searchable text, not visual fidelity.
